General Information
Office Hours
Our office opens at 9:00 A.M. to 5:00 P.M. Monday through Friday. We ask that you call ahead and make an appoiment with us if you need to be seen. We will remain open during the lunch hour during the week.
Phone Calls
We begin answering incoming calls at 9:00 A.M.. During office hour phone calls will be received by our front desk staff. They will channel the calls to the physician. If you must speak to the physician please give the receptionist your name and the phone number where you can be reached during the day or early evening. We will make every effort to return your non-emergency phone calls by the end of the same working day. If possible, please call us two week days in advance for your routine medication refills. Our schedule dictates that we must deal with non-emergency calls in clusters, opposed to stopping patient care and responding to each call as it is received.
Notice of information practices
We keep a record of the health care service we provide you. You may ask us to see and copy that record. We will not disclose your record to others unless you direct us to do so, or unless the law authorizes or compels us to us to do so. You may ask to see your record or get information about it at the front desk.
After hours phone calls
A physician is available through our answering service 24 hours a day to respond to your emergent problems; their phone number is (305) 669-9404. We ask that you use the anwering service only for urgent or emergent problems.
Emergencies
All emergencies will be seen as a priority and, as such, will be treated promptly. A life-threatening emergency requires prompt attention in an Emergency Room. Call 911 for assistance or go directly to the emergency room of the nearest hospital, which has 24-hour physician coverage.
Fees
Our fees are competitive and are based upon rates within our community. We accept Medicare, Medicaid and most insurances. We look foward to serving you with our quality health care, delivered by a well-trained, professional and courteous staff.
